The Special Populations Support Program is directed toward lessening the stigma of substance use and outreach to those populations in the community and criminal justice system. The support specialist works with HIV positive clients with substance use issues. Our support specialist is Jessica Zurwell. HIV Care Coordination as well as Special Populations Support program which deals with disease prevention and testing/education.Įrin Holbert assists with the Special Populations Program which has a testing and education component of our service. We have mobile offices with flexible hours. We offer home delivered meals, bill payer program, senior employment, information and assistance, education, resource library, case management, caregiver assistance, respite for caregivers, ombudsman, pre-admission screening for nursing home placement, point of entry for Medicaid waiver and CHOICE programs for in-home services, service coordination for children with special needs. Lifestream provides services and/or funding of services to help prevent premature nursing home placement.
